Hi John,
I assume you refer to this form since it's shown as currently unavailable. I checked the form, and its Form Status is enabled in the Form Settings, so I suggest disabling it and enabling again to see if this will fix the issue. Let me show you how:
- In Form Builder, go to Settings.
- Under the Form Status, select Disabled and Enabled again.
I have also cleared your form caches as prevention.
Give it a try, and let us know how it goes.
