I have a need to carry out a member survey (200), but wish to stop non members from using my form, each member has a number.
Is it possible to create a database of membership numbers and my form wont open unil their number matches a data base matching entry??? or is there a way that I could apply multiple passwords to the same form ??? Ie their membership mumbers.

Hi,
You can take advantage of our conditional logic show/hide condition. A two way to present it to your members:
1. Use single conditional logicfor all the members.
2. Create condition for each of your member. Since you have 200 members, you also need to create 200 conditional logic (that's a huge).
For example:http://www.jotformpro.com/form/42364992142962(Member Codes: 123).
The conditional logic:
Based on this guide:https://www.jotform.com/help/196-Hide-or-Show-Multiple-Fields-at-Once.
Thank you!
