Hi,
Have you tried re-embedding your form using iframe? That's probably due to script conflict. Embedding your form with iframe embed code should resolve the problem.

thanks. I run this via our web guys as i'm not sure how they embedded the form.
It just seems really strange as we have been testing the forms in the live environment for months with no issues, as soon as we start getting clients to complete the forms this conflict arises. I'm trying to run tests to narrow down why its just now clients are completing this.
If the iframe doesn't work the only approach I can see if removing the back/next button and having one long form which isn't visually ideal.
Have you guys seen this type of issue before?

Hi,
We will also be able to provide you more with specific suggestions and solution if we can check the actual form on the website.
Can you please share to us the URL of your website where we can at least test the embedded form?
As mentioned already by my colleague script conflict could be the primary suspect for this behavior on the form. When using the Iframe embed code of the form this eliminates the script conflict that could be happening between those on the website and those in the form.
Please inform us if the issue persist.
