I need help implementing a template on my form

Can we use this template? I need help using this form template on my new page. Kindly help me regarding this.

It looks like you're wanting to have a template to have each product fall under one column as described in the thread that you had commented on at https://www.jotform.com/answers/4028815 - is this correct?

If so, kindly note that this feature is not currently available, and my colleague has escalated a feature request ticket on the previous thread. We can certainly do the same for you on this thread, but please note that we could not provide an ETA nor guarantee implementation of this feature although we are always adding new features to Jotform.

If this is not what you were referring to, please elaborate more and we can certainly point you in the correct direction.
