Email Notification: Only can see date and Signature in emails

    I am able to send my estimate form, but when they submit, I can only see the date and signature.

    Please advise.

    Hi rvah239armory,

    Thank you for reachign out the the Jotform Support. I have checked your Estimate Form actually your form only have signature field and paragraph text. That is why Email Notification only have signature field. Default email only have form fields in it you can customize the Email Notification to add more details in it.

    Since each estimate is different, do you have a suggestion as to how I could proceed? I am trying to send procedure estimates for surgeries that require a signature.

    Hi rvah239armory,

    Thank you for providing more details regarding your concern. As you mentioned each procedure estimate would be different. Then you can use the Spreadsheet to Form Widget. In this you can load the data for forms fields from excel spreadsheet. User just need to enter the access code or unique ID and all fields with personal data will be loaded from spreadsheet. You can add the procedure estimate users data to spreadsheet. You can check this guide: How to use the spreadsheet to form widget.

    For unique code or access code. You can try to use the drop down and load users names. Users will select their name and form fields with users details and information will be filled from spreadsheet. User eill then just need to sign and submit.

    Don't think this would work for me. I would have to reenter each estimate by hand. The software I use is able to generate an estimate for a client.  What I did before was copy and paste the estimate in the text section and require a signature.  Is there any way to view the entire signed document when they return it to me?

    Hello Lori,

    You can create a PDF report containing the form data and signature and attach it to the email notification. These guides will show you how to do that.

    1. Applying Form Submission Data into a PDF Document
    2. Include Submissions as PDF Attachment in the Email Notifications
