Hi,
When we receive an email confirmation of a submission all details s present , however when we download the PDF of the form its missing 'Address' . . !
We send the info to another office and as you can imaging that one is pretty important. What am I missing??

Hi,
In your form submissions page, make sure to always check the Street Address field, it's currently NOT check:
Also, time at address - years/months fields are not checked. I think you also need that two.
