How to create form that would capture multiple information on the same field?

    Hi, I'm looking to gather information from agencies that will participate in my sport program.

    I need to be able to print each individual registration separately to appear similar to the form below. I know if I create a report it can be a spreadsheet but the data is grouped and difficult to decipher.

    how can i create a form or table for each agency to register and enter each player that is participating, provide shirt sizes from a pull down and ultmalty have it appear as the image below

    Hello rmonzon,

    Thank you for reaching out to Jotform Support. To achieve the same format as described in the Screenshot. I would suggest using the Configurable List widget. In this way, the users have the ability to add the specific quantity of players they wanted to add to the form.

    Configurable list does not allow me to create a report that shows all the participants in different cells groups them all into one so I’m looking for some thing else that each individual agency could fill out one form we can print it and Buick very similar to that spreadsheet

    Hello Rick,

    We apologize for any difficulties. The form can be integrated into your Google Sheet to consolidate all information even if used on a different form. If the form will be submitted by individual players, then that would be easier since all their data will be imported separately on the Sheet.

    However, if only a representative would fill up the form and just add their player's name. This needs to be manually edited on the google sheet. Additionally, the existing Google sheet can be used to gather data in a different form as well.
