How can I create this form table in my form?

    Hi,

    I am trying to integrate a table exactly like the screenshot attached. I want the user to be able to check box each of the rows, basically identical to this screenshot.

    Is this possible?

    That is awesome, but I would like it to not be dropdowns, and just static text where nobody can write it in. The only interactive part should be the checkbox. Is this still possible?

    Plus it needs to be multiple rows.

    Unfortunately there is no field/widget that exactly matches this. The most I come up with is the Data Grid widget and the Input Table field.

    Or use the Paragraph field to create a static table. The static table has no interactive input fields though.
