Hi, i have created 2 different forms for the website. each with difference questions and options. when they are submitted the email response seems to be merging the 2 forms into a response for one of them.

You mentioned you have 2 forms on your website, but so far I am only able to find 1 form here http://www.bickerstock.org/Bickerstock/Bands.html. If you have a second form can you please provide us with a link to where your second form is at? I could not find it.
The only thing I did find so far inspecting the page source is what appears to be two instances of the same form. You have a script and an iFrame together and I think this is probably what is creating your form conflict leading to the merged email notifications. You only need to use one. An iFrame is only usually recommended where a script will normally not work as it helps to avoid conflicts with your form and your website code.
