I have the asme issue, I have also re-added the integration and that did not fix the issue. my form URL is

Both forms no longer send data to SalesForce, they were working on 4/28/2014.

As far as I know Salesforce has been working normally from our end as we haven't had any problematic reports about it lately. Anyhow, I see you have quite a few forms and also the same with usernames. So to get a more accurate look at the problem you are having can you please tell us which of these 5 forms are giving you this problem or provide us with a link to them and we'll gladly take a look at it for you.

We did not try this - we noticed that removing and readding will cause us to reenter all mapping feilds again - Is there a way to save these fields prior to readding the integration incase we need to do this for all 5 forms?
