Hello! I'm trying to create a 1-10 rating scale. Ideally, it would use a slider but if that is not possible, a checkbox would be okay. It needs to have the ability to have labels on the bottom (ie. from extremely unhappy to extremely happy) along with number ratings. If creating a slider scale is not possible, could you tell me what type of widget is used in the employee satisfaction template (screen shot attached)? I searched for "scale rating" and couldn't find it.

You can use the Scale rating field on your form:

I think it would work but I don't seem to have that survey element. Is there a setting I need to change? Attached screenshot.

It looks like you are using a CARD FORM layout. The Scale Rating field can only be used on a form using a CLASSIC layout.
If this is the form: Meal Planner Survey, it's currently using a Card layout.
