Unable open telehealth consent form sent by my Dr’s office

Just to clarify, you cannot access the actual form or you cannot download the PDF document? Kindly elaborate more on this issue so we could provide better assistance.

My Dr’s sent the link to give my consent for the appointment and I got a “Whoops” message.
The “down load” option initially did not open
I just tried it again and got a 1 - page consent form but I did not get the option to “sign” or “agree” nor the option to return the form.

Thank you for your update.
For us to be able to check if this is a form issue, we need to have access to the actual form link. However, we can only discuss this with the form owner due to privacy issues.
What I can suggest is to try using another device to access the link and the file that was sent to you. This is the only option to check if it's a form issue.
If it is not working, I suggest contacting the form owner and letting them know of the error.
