Change header: Only given email or phone number as option

This was helpful but the only options I am give is email or phone number. I use the header name but change it to rider. I want to be able to seek my submissions with name

To clarify, are you not able to change your header name to name of rider?

I cloned your form to test but I was unable to replicate the issue. See screenshots below.

Please note that once you use Name of Rider as your header, then you won't be able to use it as your subheader as it will not be listed as one of the options when you select subheader dropdown.
