Completed Employment Application Forms downloading as Empty Veterinary Release Forms

Hi! I have an employment application form that has multiple submissions. When I download the submitted employment applications, the file name contains the applicants name, but the pdf is an empty version of another form of mine, the Veterinary Release Form.

I can open and view the Application Forms submissions, and they look fine, but when I download them, the pdf file name shows that applicants name, but when you open the pdf, it is a Veterinary Release form with no fields filled in. I can't figure out why this is happening. It makes no sense. Help please!

Thank you for reaching out to JotForm Support! I just cleared your form cache, and would ask that you take the following action to have this issue resolved:

When you attempt to download this form, can you please click "Customize PDF," rather than "Download PDF", and use our PDF editor to build this PDF out to your liking?

Once you have your new PDF Build, you can proceed to the PDF editor tab of our dashboard, and delete the original PDF that is being downloaded in the incorrect format. Once this PDF has been deleted, your downloads will default to the new format that you create through our PDF editor.
