I have set up several forms Using the Approval flows in the settings menu.
They do not seem to be working is there a step I missed or is this not the correct way?
Once the initial approval is completed the second manager does not receive the next email.

I have tested the Approval Flow feature on my end and it appears to be working as expected. I checked the form you shared and from what I understand here's the same workflow you have constructed.
I have made some test submissions and the email intended for the second manager was sent as expected.
When the form is submitted, the first manager will receive an email similar to what is shown in the image below.
And whether the first manager click Approve or Deny, they will be redirected to your form's Inbox page.
Based on the Approval Flow, if the first manager selected Approve,
the second manager's approval will remain pending and the second manager will receive an email.
Again, whether the second manager click Approve or Deny, they will be redirected to your form's Inbox page.
If both managers completed the approval process, the submission will be moved from the Pending Approvals tab to the Completed Approvals tab.
May I suggest that you try using a different email address for the second approval process, please?
