Thank you for contacting JotForm Support. We do apologize for any inconvenience that you have experienced.
After checking the form you have shared with us, I found that you have set up the Unique Submissions to "Check Cookies and IP". Please note that the Unique Submission setting is used to prevent duplicate submissions.
It can check for cookies to prevent submissions from the same browser, IP address to prevent duplicates submissions from the same device or internet connection, or it can check for both. In your case, you have set it for both. This is why you are unable to submit more than once. If you try using your form from a different device, you will be able to submit it one time again.
You can easily turn this setting off so you can allow multiple submissions. Here is a very helpful user guide for assistance - How-to-set-unique-submissions-on-a-form
