Hello!
I was setting up a form to start using your service again and my account has been suspended
My username is: fooorex
I don't think it has guilty fields since it don't hask for any detail but name and email, an id and password are generated by the form itself to access the member area, it does not collect Passwords from the users

The form, MillionN.com (https://form.jotformeu.com/91640733044351), was found on your account (fooorex) as suspicious of phishing activity because you're collecting user credentials.
Gathering this type of data violates our Terms of Use.
Asking for an access code to conditionally show/hide fields within your form is okay. However, you should refrain from using "password" as your label to avoid being flagged by our phishing detector. Kindly change it to something similar, like an "access code". Other than that, no other kinds of passwords or user credentials are allowed to be collected.
We will only lift the suspension on your account if you agree with our terms and allow us to delete the offending field mentioned above.

That filed is hidden and it does not collect any information, it creates a random code that is used as a password in addition to the id that is also generated by the form
I agree to terms you can delete it i'll replace with access code
