Why does the auto-response email not show the Scheduled time? Very confusing for form users.

I create a clone of your form with 2 tests and I found that the Appointment field is not required, so if the user didn't select a date the email is not going to show the empty field.
Please make it required, so the user can receive all the data.

I changed it to required and the result is the same.

I was able to reproduce the issue you are having with a cloned version of your form. It seems that even if the Appointment Field you are referring to is included in the content of the Email Autoresponder in your form,
the data that was submitted on the Appointment Field does not appear when the email autoresponder is received.
May I suggest that you try setting up a new email autoresponder on your form and then test your form again after, please? Please do not hesitate to let us know if the issue persists after you do that. If the new email autoresponder works, you can already remove the old one that does not work.
