Some of the areas in my form, like the phone number and the reason for visit, you can't see when you are typing in the information. It looks like nothing is able to be typed in that area. But when I submit the form, I can see that there is text typed there. How do I fix this?

I checked the form, and it seems that the fields with issues had their default font color set to "white" so they are not visible.
However, I made customizations and they are now visible:
If that is acceptable, please insert these codes into your Inject Custom CSS area:
input#input_8_area {
color: black !important;
}
input#input_8_phone {
color: black !important;
}
input#input_9_area {
color: black !important;
}
input#input_9_phone {
color: black !important;
}
textarea#input_15 {
color: black !important;
}
textarea#input_22 {
color: black !important;
}
