HIPAA Compliance: Can JotForm sign company's BAA?

Hello:

I am a graphic designer who developed a WordPress website for a Behavioral Therapy Company. They are looking for HIPPA-enabled forms for their new client intake packet on their website.

My Client wants to know if JoltForm will sign their Company's BAA. They are looking at purchasing the Silver Plan from JoltForm within the next few weeks.

Thank you for contacting us.

Unfortunately, we do not sign individual BAA's. However, with the HIPAA Compliance, we provide our own BAA.
The BAA is provided after upgrading to HIPAA, but you can find a sample of the BAA from the link below;
