Hello! My form says it is disabled and a customer is trying to place an order. Can this be fixed?

Your form and account are disabled/suspended due to the usage of the word "cash app" in your form.
I have activated your account now. Could you check if you can remove this word from the payment type dropdown at the end of both forms in your account?
