Hello,
This is due to the settings of the form wherein the Continue Forms Later is enabled, and you had filled in the form before and automatically saved the incomplete session. Here's a related guide: setting-up-the-continue-forms-later-feature-in-card-forms
You can check the draft saved on this form, through the My Forms page under the My Drafts folder. You can either discard the draft or continue filling the form:
