Hi support,
Sorry English isn't my main language. So, I post my problem on this page. https://www.jotform.com/help/442-how-to-use-the-spreadsheet-to-form-widget?if_id=442 (id hrlmw).
My form https://www.jotform.com/build/202031916491045
My problem file http://www.mediafire.com/file/986j5nf8lg26yhs/letter.xlsx/file
Usually the widget will populate data from the excel file and fill my form. Now it's not work and you can try delete and reupload it.
I'm not sure that your widget has limit maximum size of excel file or not?. Then I try recreate another file with same columns structure like this http://www.mediafire.com/file/0fivzou3cwxxwx7/Book1.XLSX/file , then upload to form and it works.
Please help and thanks alot,
Sirapat

I can see that the file contains a number of worksheets with a large number of additional columns.
The excel file that you upload on the widget should contain the exact form field labels as column titles and should not contain formulae.
Then the fields will be filled up using the widget.

This file worked very well with this widget for 4 months with these worksheets and columns until last Monday.
This's not first time with this widget.
What happened with my file and this widget? And I read on your forums, not only me that has this problem on the same time.
Why?

We are sorry for the inconvenience this has caused.
We were experiencing a temporary issue on Oct 29th which was solved in some time on that day only. The users also confirmed.
I see there are 2 erroneous columns in the Excel sheet.
Also, you should have only a single tab on the Sheet. Delete all the extra tabs on the form:
Then re-upload the Sheet on the form.
Please give it a try and let us know how it goes.
Thanks.
