Does the patient have to create a JotForm account to fill the form out?
The email that the office staff will receive has all information hidden! How can I make this info appear?
Does the office staff need to login to JotForm to get the Submissions?

> Not necessarily, if the form has no access restrictions, anybody can fill out the form.

> I check the account associated with email address: vreitz@cgicompany.com and it is under our Corona Responders Program making it HIPAA-friendly, along with its data protection. Only the form/account owner can access the submission data.
Does the office staff need to login to Jotform to get the Submissions?
> Yes, but they need to access the Account where the filled out form was created from in order to view the submissions.
