How to create card forms?

    Hi there,

    I am new to JotForm and I am trying to find a form template to start with that I saw on a few landing pages. However, I did not find that very one inside the library in my account (please see attached).

    Can you please put me in a right direction in the forms library and help me to locate this form type?

    1) Remove blue background and green button.2) Remove the Welcome page - just start from the question card.3) How to insert First name token in the question. How to personalize?4) How to customize the "This filed is required" when someone tries to skip the question?5) How to make sure they goto the correct next question based on what they have chosen? For example, if they chose the phone call they need to go to the "What's your number" question. etc, (see the video for details)

    1) Remove blue background and green button.

    You may customize the colors of the colors for the background, buttons from the Form Designer. Please see the screencast below:

    2) Remove the Welcome page - just start from the question card.

    You may remove the Welcome page by clicking on Remove Welcome Page button displayed on the top right corner of Welcome page card.

    3) How to insert First name token in the question. How to personalize?

    You may use the Unique name from the Name field properties to add in the question label of the other fields. Please see the screenshot below:

    4) How to customize the "This filed is required" when someone tries to skip the question?

    The error message or warnings can be customized from the form settings.

    5) How to make sure they goto the correct next question based on what they have chosen? For example, if they chose the phone call they need to go to the "What's your number" question. etc, (see the video for details)

    You may setup conditions to show/hide the fields or skip the fields based on the selection on the form. Please see the guide below for more information on setting up conditions.

    thanks so much for the replies and the screenshots! 
    I have implemented all the changes except for the Unique name token. I am still not clear where to access and get these personalized name tokens. Do you mind showing me in more detail like screenshots or video?
    And lastly I made some tweaks in the conditions part and I guess for my case it should be Skip to a page feature which I used . However, I wanted to double check with you if this is correct settings. 
    Basically I am asking what's their call preference and if the answer is either video or audio of Zoom then I want to ask for phone number also. If they select phone call only do I have to create a new step for phone call only? Please see below the screenshots.

    Instead I need it takes to other page of the funnel. And I see you don't have integration with Calendly? Can I link the form to my calendly calendar?
    And lastly, is there a way to lock the the steps below so they were not able to jump forward and back. So they answer one question at a time until they get to the end? I want these dots in the bottom to be removed? How can I do this?

    I think you understand it correctly, but you only used the wrong condition for it. You will need to use the show/hide conditional logic instead of SKIP TO condition. Please check my screencast below for reference if you want the user to answer one question at a time.

    We do not have direct integration with Calendly, but you can use Zapier as the medium so you can integrate JotForm with Calendly.

    Hi there,
    1. Condition: thanks for the reply. I will change the condition then. Should I remove the current one I guess? Do you mind take a look and see if the condition is right this time? 
    2. Personalization name token is not working again {hiThere} It used to work fine for some time. But I noticed at some steps it is blank it is not pulling the name again. What's the catch here?
    3) Zapier integration: So the way it works after they click submit button thanks to integration with Calendly through Zapier they will be taken to the URL of my calendar? What I need to happen is when once complete the survey they click Submit and they are taken to this page https://gravityfunnels.com/schedule-page. Do I still need to use Zapier? I think I miscommunicated in my last email by saying that I need integration with Calendly. I want them to be redirected to this URL https://gravityfunnels.com/schedule-page after they Click "Submit". 
    4) Dots in the bottom: You missed my question from previous email is there a way to lock the the steps below so they were not able to jump forward and back. So they answer one question at a time until they get to the end? I want these dots in the bottom to be removed? How can I do this? Here is an example of the page I want mine to be like this. 
    5) Also what I noticed in some spots, e.g. in my office all types of jotforms cannot be opened. It's not only with my form it's across all your clients forms as well. Do you know what could be the reason why? I have to turn off wi-fi where I am right now in order for me to be able to see the form on my phone data.

    2). I just tested your form and it's passing the name value.

    3). Just add this URL https://gravityfunnels.com/schedule-page on your thank you page https://www.jotform.com/help/499-How-to-Edit-Thank-You-Page-Settings.

    4). You can set the "Show Form Progress" option to "No" in form Settings to remove the dots.

    5). That's a possible firewall restriction. Try using a different ISP or internet access outside your office and see if you can view Jotform.

    Awesome! Thanks so much for your continued help, Jed. Appreciate it much. 
    As a next step I plan to integrate with ActiveCampaign and I am curious if the integration works only for the AC CRM or contacts can be added to the automation in ActiveCampaign as well? 
    What I am trying to achieve with integration with the AC is that once a contact gives a name and email address they need to be added to the corresponding automation in AC. Therefore, by integration Jotform with ActiveCampaign will I be able to add all these people into the pre-build automation in ActiveCampaign?
