I would then like to calculate a total cost basedon the choices made. The calculation would be based on a percentage of the employee membership cost. I just can't figure outhow to do this.

Hello Bob! I cloned your form and used 'Input Table' field instead on 'Configurable list' widget. I think this should solve your purpose.

Based on the solution, provided by our colleague Patrick on your other ticket, it is possible to calculate the total cost.
What you need to do is to assign value to each radio button on the input table on the form https://form.jotform.com/202345467734963. Then use a Form Calculation widget to calculate the total.

However, if you can clarify this sentence - "The calculation would be based on a percentage of the employee membership cost." - we can come up with a demo video and form to suit your requirement.
From where do you get the employee membership cost. Is it added by default on the form (do you show that amount on the form) or entered by the user?

I have decided to simplify it by only including the Employeeas a choice for selection. The percentage refers to the cost of membership, which I have not yet included on the form. I knowthat it needs to be included on the form to be use as a part of a formula for calculations.The Employee Membership is $360 annually. I want to be able to allow the Employer to calculate or do some "what ifs"about his cost of subsidizing memberships (what percent he would be willing to contribute, for instance). There should also be aplace for the employer to enter the total number of employees involved (this field would be used in the calculation of total cost).
Based on the number of Employees, the annual cost of the membership and the percentage chosen by the Employer, I wouldlike the form to calculate the Total Cost to the Employer both monthly and annually.
Some of the features I would like in the form are: - To have the table only appear if the Employer chooses to subsidize his employees memberships - The ability to clear the percentage choices radial buttons that were clicked on (for what-if scenarios) - To have the calculations clear (clear button?) or be replaced by new calculationsGenerally, I would also like to know how to: - Reposition an element on the form horizontally, if it is possible. That way there could be to smaller elements on the same line. - How to adjust the element boxes to be closer vertically, if possible. I know I can shrink them, but that only seems to apply to the width.I sincerely appreciate your assistance.

I appreciate your help. How can I see the actual form builder version so that I can learn how to do this myself and see how theform was built? I would love to understand how you were able to put elements side by side and how to locate them on the page.Also, I would like to understand how the calculations were set up. Lastly, how is the bottom half hidden until the radial button osclicked on?
Lastly, when you clone a form to put it into an account, are you using the URL of the form?

Hello,
You can clone the form that was created by my colleague.
Once cloned, you would be able to see the form builder.
All the conditions and calculations would be visible.
For the bottom half, it was set up as a show/hide condition:
Kindly clone the form and check the demo form and let us know what revisions are needed.
