Configuring payment form with credit card processing

My form is coming together. However, I have some questions. 

1. Under "Spouse" membership ... how can I add a field to enter the Spouse Name?

2. We would also like a "Donate" option with the words ... In addition to your membership fee, we appreciate additional donations to CRWF to further our programs and activities.   How do I add?

3. I've got the credit card information loaded ... how do I take it active?

Unfortunately, this is not possible. You can add a field below the product list and show it when the spouse membership is selected.

2. We would also like a "Donate" option with the words ... In addition to your membership fee, we appreciate additional donations to CRWF to further our programs and activities.   How do I add?

You can add it as a separate product with a $1.00 price and a quantity input.

3. I've got the credit card information loaded ... how do I take it active?

If you would like to process payments through Square, you will need to connect the product list to your Square account.

Or, you can select and connect another payment processing integration.

Thanks Mike for your email. I was able to adjust #1 - add Spouse Name and
#3 - I will work with our treasurer to assure Square is connected.
Regarding #2 - Donate option. Just to be certain I understand. No specific,
variable amount can be added, i.e., $50 or $100 or whatever someone wants
to donate? It seems like we could have up to 10 FIXED amounts - right? And
each fixed amount could be listed. Thanks again, Mary

Yes, the amount in the Donation is fixed as it is added as a product in the listing.
