How to create a form based on a website

Please provide a form I can clone that looks like the form and colors

Thanks for contacting us.

So, unfortunately, we cannot build you a form from scratches. But we can give some instructions. 

First, you can check our guideline, to help you build a form here: How to Create Your First Web Form

Then you can add your logo on your form as I did on the picture below by using the Logo Properties

After you can also set up the main colors of your form, which are mainly dark from what I can see on your website. You can click on the Form Designer > Color Scheme and from there add the color you want. 

You can also use the picture widget that will allow you to add the same pictures as your website. 

Once you are done with the design you can add the fields of your choice, regarding the purpose of your form. Here you can check our instructions: How to Position Fields in JotForm

I dont need to add logos and images to the form.

Just the 4 fields with the colors to match.

I tried that form designer....did not work well for me.   I have have gotten support before that provided a link to clone.
