Account suspended due to a Phishing form

In this case, the account has been suspended because the form https://www.jotform.com/form/201961446460454 is asking for Credit card information:

https://www.jotform.com/terms/

Phishing. You agree that JotForm Inc. may terminate your JotForm Service immediately if a form’s purpose is found to be deceptively obtaining, for example: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

Please reactive back my account with delete the offended account. I will create new form without this information request.

1. Company name

2. URL where the forms are embedded

3. Corporate email, you will have to use your corporate email within your JotForm account instead of your current Yahoo address

4. Proof of affiliation.
