Hello,
I can see from your PDF form, that you have added few text fields and it has created few field areas on your PDF form - right side.
What you need to do is first delete these mapped fields and field areas and then add each form field individually.
Example: Name, email, Phone, etc. As soon as you add these form fields, it automatically creates a field area and gets mapped to your PDF. You just need to drag and resize them to correct locations.
