I have a WordPress website with a form here; https://premiumwebsites.net/contact-dotty/
The form showed until I filled it out - now when i return to the page I get a brief flash of the form showing then it disappears.
Is there a setting that controls showing the form for people that have already filled it out? How do I shut that off?

Could you please re-embed the form using the iFrame code:

difference. I also cleared the cache from the server thinking that might be
the problem.

Hi, are you using Internet Explorer browser? I checked your site in Chrome, and Firefox, and the form showed up correctly. However, it did not show up in IE 11:
Please try embedding the Iframe code without the script part, if you paste the code in a text editor first, you will notice the Iframe part only, example:
Please make sure to set the scrolling attribute to "yes". Let us know if you need more help.

Sometimes, there may be issues with the default JavaScript code, this is why we provide different embed methods such as the iFrame which helps to prevent this kind of issues.
