PDF converter have some incorrect or missing fields

    Hi,

    I encountered two issues when I try to finish building the form:

    1. missing data field (eg. can't find patient first or last name or address on the left side)

    2. in the middle section, how could the patient choose Y / N for these conditions?

    Since PDFs are created on an entirely different application, JotForm's PDF Converter simply scans your PDF and checks for clear and identifiable input fields. At times, the PDF Converter may fail to identify your PDF fields.

    Since the PDF Converter knows these limitations, you can actually add fields within your online form and map it to your PDF form.

    Do you have any suggestions on the best way to set up the Y/N answer?

    You may use Single Choice field for Yes/No questions.

    However, looking at your PDF, I'm afraid the best you can do is put the mapped box to the side of Y/N text of your PDF.

    Or you can also use an Input Table field. In this case, it will sort of add a block to the PDF field as well, just make sure to resize the block so it will look more neat.
