Form Scheduling

Hi,

I'm trying to use google forms for scheduling my appointments. I want my patients to be selecting date and time through the online form. I'm not sure how to do this with online forms and come across your website while doing research. Hope you can help.

You can create a google form with a multiple choice field that shows time slots for appointment times. You need to have an add-on - choice eliminator - which will allow you to unable a time slot if it's previously taken. But I think your goal is a little more than that. If you need to schedule appointments, JotForm is the perfect tool for you. 

With JotForm Appointment field, you can when, how long, and how often your appointments will be. You can always limit the number of users who can signup for a particular time slot. You can avoid conflicts by setting the appointment type as one-on-one so that a single user can attend the session. All details such as date, time, duration as well as full name and email address will be stored in your JotForm account. Therefore, you'll have more control over your appointments and will be more organized.

Unfortunately, we are unable to help you with Google forms, but we will be happy to assist you to create such form using JotForm.

I would suggest you use the Appointment field for this. With this field, you can set the duration for each appointment, and the time and dates between which your users can make a selection.

When they do, that field will not be available for other users.

You are, of course, able to add different fields next to the appointment, like name, email, phone, etc.

After you create the form, you can connect it to your Google Sheets and Google Calendar to keep your records.

Every time a form is submitted, you will get an email notification where you will be able to preview the user's answers. Their submitted data will also be saved to JotForm servers as well.
