HIPAA Compliant Chiropractic Intake Form

    I'm considering to upgrade our account so we can start using online HIPAA compliant forms. I have 9 forms, but three of them are very complicated. I attached one of them so you can advise how I can create this form in JotForm. The particular area of concern is the diagram section where the patient is asked to indicate level, place on body and type of pain.

    And second, how do you handle legal signing of forms by the patient?

    Please advise how we might solve this using JotForm HIPAA level account.

    You may use the annotate picture so that you can upload an image and the user can write in the image.
