What's the Best Form Template to use?

Hi, I would like to create a Service Agreement jot form. Exactly the same as the mock-up I have designed. Please help me look for the best free template

Hi there, thank you for reaching out.

Based on your mock up, you can start with any of the following free templates.

Big

Busy Tech

Autumn Forest

Greenlake

Now, just proceed to "Advanced Designer" to further customized your form by uploading your own form and page background, changed fonts style and colors and basically meet the design that you prefer.

We don't have an exact same template you have. As what my colleague has shared, you may start from any templates and add your background or CSS to replicate your original template.

Can i transfer my jot form to a different user?

Yes, we can move the forms to another JotForm account user.

You may follow this guide and create a new thread to request the form transfer.
