I have tried to use this widget and I select the unique key. It says "Fields Have Been Auto Filled" but I check the fields that are labeled with the column headers and nothing shows up. Did I miss a step in the setup?

Could you please ensure that form labels is same as the column name in the Spreadsheet. Also, they are in sequence.

I have checked and the column names and the form fields are labeled the same. The form fields are also listed in the same order as they are in the Excel Sheet.

I sent a replay through email as requested and have not gotten a response to this issue. I ran a test again today and a couple of the form fields filled but the rest didn't. Any solution to this?

To verify, are you still referring to this form, 200637636093153? I checked all your forms but I can't seem to find the fields that match the columns on your spreadsheet.
Kindly provide us the form's link and fields you are trying to prepopulate using the spreadsheet?

Yes that is the one that I am referring to. The columns I am looking at are:
Name
Salary
Action
Comedy
Drama
Hype

Thank you for the additional information.
It's because of the colon (:) on the field labels that are not on the column labels of your spreadsheet that is causing the issue.
The Column Names on your sheet should match the Field Labels on your form.
Please try to add the colon symbol on the spreadsheet or remove the ones on the form, it should fix the issue.
