We apologize for the inconvenience caused. I checked on your account/username pdawgconsulting and I noted that it was auto suspended by the Phishing system due to the suspicious phishing form that uses a copyrighted logo in the form.
Since your JotForm account profile does not show an email address related to it, this is violation of our Terms of Use.
Please change the account profile email address to one that matches the domain with logo and also the corresponding terms/agreement clause from the company into the form , or in order to reactivate the account, we should remove the logos on your forms.

We can delete/remove the logo on the suspected forms after you agree deleting them. And you must read and comply to the Terms.
Let us know here if you agree to deleting the logo now and we will proceed.
We can reactivate the account then after.

We have now unsuspended your account and deleted the mentioned logo. If you can provide us any proof that you're one of their vendors, we may be able to allow you to keep the logo.

I deleted the documents in the account and then uploaded a new document
without the logo. My account is suspended again :(

I would suggest you to please purge the form #200455437044045 from trash folder. I would also suggest you to please use your account profile email to corporate domain.
