Hi I just signed up, built a form and my account is Suspended??? Can you fix this please? I was enjoying jotform until this.

Hi Stephen - As checked, your account has been auto-suspended by our system as this form: Lynx Client Onboardon contains plain text fields that collect Password/Login credentials. It is considered as phishing and violates our Terms of Use.
You agree that JotForm, Inc. may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining, for example: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.
We can reactivate the account but we need to delete the infringing fields from your form. You must also agree with our Terms of Use.
