can I include pictures above each form questions if it is one form per page?

Hello,
If I understand correctly, you would like to include an image to a question filed. Unfortunately, there is no image upload feature to questions. However, you might use some of our widgets to serve a similar purpose.
You might use the "Text" tool and include both images and text to one filed.
You might try our Image Choices Widget.
You might use the "Image Choice" field.

If I understand your query correctly, you want to display an image above the field in the card layout form. If so, unfortunately, it is not possible since the card form layout is intended to have one field per page and the image is also considered as a field.
You may consider using classic layout and add page breaks to create a multi page form that might help achieving your requirement. You may apply/create a theme to the form as per your requirement.

You may switch the form layout from the Form Designer easily. Please see the screenshot below:

I hve done this but I can’t get the thankyou page to come up and I can’t make the last section have ‘finish’ at the bottom it just says back.
This is because there is no Submit button added to the form. Please try adding the Submit button to the form from the Form elements so that you should be able to submit the form.
The question related to Terms widget is moved to a separate thread and shall be addressed there shortly.
