How do I create a Safety Form?

Good Afternoon, 

Could I please get some information and a quote to build a simple “COHO Safety Form” app for the attached form. 

We want it to be date and time stamped to ensure the workers are doing the form, and that it will upload to us in the office and the worker's supervisor.  We want to be able to track and trend the number of forms being done, so we need the ability to graph and track by client, by worker, track overall form submissions etc.

You can use the Date picker element to capture the date field.

You can use the Submission Counter widget  to capture the number of submissions received so far:
