I have tested a cloned version of your form and it seems to be working correctly at my end.
To clarify, may we know if you have setup your own SMTP in sending the verification email?
I have cleared your form cache.
Can you please test your form and let us know if the issue still remains?

Could you please delete the SMTP sender address and configure it once again.
Were you receiving the emails when you set up the sender emails as no_reply@newidiabank.in?
Let us know how it goes once you re-configure the sender's address.
Thanks.

According to your response, i have configured SMTP Sender Address again.
Also trying to make one dummy entry but still its giving the same error
message.
Is there any possibilities of blocking Gmail account from Jotform.
Kindly look into it.

I would suggest using the default configuration and remove your own SMTP in sending the verification.
You can block Gmail emails by using conditions.
You may use the state contain and hide the submit button if the email contains "@gmail.com".
Sample condition:
