How can I create a wholesale order form?

Hi, I have attached a screenshot of what I am trying to achieve.  As there are multiple products which come in multiple sizes, it is important that the form can total up horizontally and vertically.

Do you have a template that I can base this on or is this even possible?

You can make an order form with Jotform. It also allows you to integrate your form to payment processors listed here https://apps.jotform.com/category/payment_processing.

When you create products you need may create a sub-product from your options.

For example, you can edit the products by clicking on the pencil icon.

For every product you added add quantity selector as well as Product Option. In this case, the product will be sizes. 

Now enable the option "Create Sub Products from Options" and enter sizes as shown below -
