Good Morning,
I attempted to login to my Jotform this morning and realized it had been suspended.
I wasn't sure why this had happened and did some research online and realized that I had created a form for clients to order reprints of our product. I had created a section for them to provided payment information and I now realize this is against the terms of service.
I apologize for the mistake, I did not know this was not allowed.
I was hoping someone could help me reestablish the account so I could continue to try the services.
Could you please help me? -

We checked your form then we saw that there are many fields to get some bank account information at your form (Form ID:92824302421145). This situation is against our Terms of Use. If you let us to delete these fields we can activate your account.
