Hi there,
We do not have this feature in our form builder, but a workaround can be made. It may not a perfect workaround, but at least you can show the result. I would suggest using our classic form layout you can better customize the form including the design.
https://www.jotform.com/help/488-How-to-Change-Form-Layout
So, the method would be the following:
1. You'll create a multipage form. One question per page. The page break will be your next/skip buttons.
Guide: https://www.jotform.com/help/119-How-to-Create-Multi-Page-Forms.
2. In each page, add the image and then the options. You can use our Single Choice element for the answer options.
3. To get the result, it is either you will calculate the answer with the calculation widget or configure the form with our Quiz app.
Quiz app guide: https://www.jotform.com/help/289-Turn-Your-Form-into-a-Quiz-or-Exam-.
Here's a demo form you can check - its format: https://form.jotform.com/92394600433959.
The only thing that's not possible here is the random order or random colors/numbers when you refresh the page.
