Can I add to your suite of existing clinical forms and questionnaires?

Hello,

Can I upload behavioral health questionnaires, such as the PCL5, ASRS, and other such forms to Jotform to use at my clinic? I represent Comprehensive Clinical Services, P.C., a private practice outpatient mental health group with locations in the Chicagoland area. My clinical team is looking for a solution to administer questionnaires to patients in a way that integrates with our EHR system, athenaOne.

Since patients will be filling out these forms, you'll likely need to ensure compliance with HIPAA (Health Insurance Portability and Accountability Act). This can be done. You need to be on our Gold or Enterprise plan to enable it. Check out our plans here. When you enable HIPAA compliance, we'll move your data to our HIPAA server, which provides high level security.
