Hello Details and Pieces,
I cloned your form and tested it to see if I could replicate the issue, but everything was working properly. I tried enabling one of the items in the Product list, added a sub-option, and once previewed, the test option appeared right away. Check out the screencast below to see my results:
Can you try it again and see how it goes? If you run into the same issue, please share a screen recording of what's happening on your screen then upload the recording to any cloud-based storage like Google Drive and set its shared link to Public so we can access it, and share the link here then we will try to replicate and investigate the issue further.
