Broken thank you page

I'm receiving strange "Thank you page" after the submission

It should look like this

I also checked both the Notification Email and Autoresponder Email as they have been included in your screenshot, but both of them also shows as expected. Please check out the screenshots below for my results:

I've cleared your form's cache which should help. Can you try it again and see how it goes? If you run into the same issue again, let us know, and we'll do some more testing to see what's going on.
