How to get multiple signatures?

    What is the best method of providing the facility to collect between 3 -30 signatures on a Work Prestart Checks form? All crew members will sign onto the form using a signature block. Crew sizes vary day to day.

    Hi ntelife_Forms,

    Thanks for reaching out to Jotform Support. If you need to collect multiple signatures you can use our E-Signature Widget. You will be able to add multiple E-Signature widgets on the form to collect multiple signatures. Let me show you how:

    1. In the Form Builder, click on +Add Form Element.
    2. Click on the third tab Widgets, then ;ook for E-Signature and drag it to the form.
    3. Click on the Green Pen Icon and turn on Sign Automation to send the email.
    4. Repeat the process for the second Signature.

    Thanks Israel no need to send out request for signature all crew members are sat in same location will simply need to come up to tablet and sign with their finger or stylus. In fact we couldn't send it out as crew members do not have access to any devices to receive the request. Outcome is we have a form on the tablets where we can enter crew members names in a list and ask them to sign against their name. Will be between 3-30 people.

    It seems like you're working on the Clone of Rail Safeworking Pre-Start Brief Site Compliance form. When I inspect the form, the fields you added already meet your requirements. What I can recommend is you add is field which will identify how many set of questions will show depending on the selection: Let me show you how:

    1. In Form Builder, click on the Add Element button.

    2. Search for the Dropdown and add it on your form.

    3. Click on the Gear Icon and go to the Options tab and add the number of crew members.

    Next you will need to set a conditional logic to show and hide the correct fields based on the selection.

    1. Click on the Settings tab at the top, then go to the Conditions tab on the left side.

    2. Click on the +Add Condition button and select Show/Hide Field element from the options.

    3. Set similar conditions, that will show corresponding fields base on the number of crew members.

    Thanks Kris Ann your idea is useful. It doesn't really meet the requirements though.


    There's a common scenario where a risk assessment is carried out to identify potential risks when undertaling works. Controls are then agreed to mitigate risks Anyone that is involved in doing the work signs the form. The work may span over several days or even weeks with workers coming and going.

    It would therfore be useful to have a Widget that includes ability to capture the signature (at the work site via tablet or mobile) and have the ability to keep adding another row as and when required.
