Need suggestions on which templates to use

    Good afternoon!

I am new to JotForm and I need a little guidance please. I'm having a difficult time deciding which template will best suit my needs.

I need to create a form for clients to be able to purchase a low cost spay/neuter voucher. In addition to the usual client information fields, I need the client to be able to upload a photo showing proof of need (photo of government assistance, etc). I then need the client to be able to pay us.

Can you suggest which template which might be best to start with? I love that you have so many to choose from but admittedly the sheer number of templates is a bit overwhelming.

    Thank you.

    Hi Kay,

Thanks for reaching out to Jotform Support. To better assist you in selecting the ideal form template, can you let me know if you have any specific payment processors in mind (e.g., PayPal, Stripe), and how you’d like the proof of need documents to be handled or stored? This will help me recommend the best workaround for your form.

After we hear back from you, we’ll have a better idea of what’s going on and how to help.

    Hello,

Those are a couple of good questions!

I have no idea regarding how to handle or store the proof of need documents. I've never even thought about it. (Like I said - I'm a total newbie! lol) I'm certainly open to suggestions because I don't even know what options exist.

Regarding payment options, we need to be able to accept PayPal, Venmo, and CashApp, at a minimum.

Also, at least 50% of our clients will be using the form from their phone, if that matters.

    Thanks!

    Adding to my previous reply...

Our primary software we use for the nonprofit is ShelterLuv, which has a text-to-donate function as a component of ShelterPay. Our accounting software is Aplos, which I'm still learning as well.

    Thanks.

    Hi Kay,

You can use the File Upload element feature, which allows clients to submit their proof of need documents directly through the form. These files can be stored securely in your Jotform account and accessed anytime. If you need more advanced document management or integration with your existing systems, you might consider connecting Jotform with cloud storage solutions like Google Drive or Dropbox, which Jotform supports. Jotform templates are also responsive and designed to work well on mobile devices, so your form will be accessible and user-friendly for clients using their phones. About your question about PayPal Integration, Jotform supports PayPal integration, which can handle payments directly through the form. About your concern about Venmo and CashApp, while Jotform does not have direct integrations with Venmo or CashApp, you can set up a custom payment option where clients can select their preferred payment method and provide instructions for payment. Unfortunately, Jotform does not have direct integrations with ShelterLuv or Aplos, but you can export form data to CSV or Excel and manually import it into your accounting software if needed.

Let us know if you have any other questions.

    Thank you.

So do you have any suggestions for a template I might start with for the voucher application?
