As you may know, Jotform is an Online Form Builder where our users are able to create and use their own online form for their work, business, education, and more as long as they follow Jotform's Terms of Use. Can you explain a bit more about the issue, so I can get a better idea of what's happening? It would help us to understand what’s happening if you could take a screenshot and send it to us. I am sorry.. I returned a CPAP machine (equipment) to APRIA, then I get an email from 'jotform'. Thus, I do not understand why.. is it that you provide the labeling for them?..

I need to know If they received back the equipment ?

I assume you have filled out one of our user's forms. Please note that Jotform is an online form builder where our users use our platform to create forms that can be filled out online and are not affiliated with them. I suggest you contact the owner of the form thru their website or wherever you found the link to the form you have just filled out.
